When in need of at-home solutions, how can we help?

Today’s society is faced with the challenge of maintaining high standards of health care despite a continued increase in medical costs, population of older adults, and an ever-increasing number of people diagnosed with a chronic illness that require long-term care. Most want to continue living independently and only require assistance when needed, but the majority in our community do not have the social/financial support to obtain an at-home companion.

With the rising standards of health care and expensive medical costs, it has become increasingly difficult to obtain proper care. Through Pathways’ Care Navigation program, however, individuals can find help and comfort with our Care Team and compassionate volunteer companions.

Initial Assessment

First and foremost our program centers on building a relationship with the client.

To best understand each client's needs, a member of our Care Team will meet with the client and family (if requested) to perform a detailed, comprehensive assessment in the comfort of the client’s home. During the assessment, our Care Team will:

  • Gather detailed medical information

  • Document medications & assess if intervention is needed

  • Assess living situation, home safety, and support system

  • Assess physical and cognitive status

  • Discuss advanced directives (healthcare power of attorney, living will, etc.)

Upon completion of the assessment, we will return to the office and develop a Plan of Care. This plan will include professional observations as well as client, family and other involved parties' input. Preferences and recommendations are put into the detailed Plan of Care or “action plan”.
